A Mandibular Advancement Device (MAD) is an oral appliance that shifts the lower jaw (mandible) forward to help keep the upper airways open during sleep. This positions the tongue forward, improves airflow, and prevents snoring and pauses in breathing.

The MAD is used to treat Obstructive Sleep Apnea (OSA) as an alternative to the CPAP machines traditionally used for this condition. It is also suitable for individuals with chronic snoring. However, it is not suitable for all types of apnea; other solutions must be considered for central sleep apnea or mixed apnea.

It consists of two splints—one for the upper jaw and one for the lower jaw—connected by bars, screws, magnets, or another fastening system (depending on the manufacturer). These push the lower jaw forward, thereby shifting the tongue and soft tissues of the mouth slightly forward as well, allowing air to circulate more freely through the airways.
